Position Overview
$2000 Sign-on Bonus
In this vital community nursing position, you will serve as a Palliative Registered Practical Nurse (RPN), conducting scheduled visits to provide care for patients in diverse environments, supported by Registered Nurses for more intricate cases. You will engage with patients across all age groups, addressing a variety of care complexities while playing a crucial role in ensuring their comfort at home for as long as feasible. Work Schedule: Full-time & Part-time options available. Evening shifts only, from 2 PM to 10 PM, with every third weekend off.

Key Responsibilities:
- Conduct scheduled community visits to address patient needs within your professional scope
- Deliver ethical and safe nursing care
- Utilize appropriate assessment tools and techniques to evaluate individual patient needs
- Implement and continuously monitor care plans, making adjustments as necessary
- Engage patients through a collaborative approach, fostering rapport and trust
- Maintain and update professional patient records related to encounters and care plans
Qualifications:
- Registration with the CNO as a Registered Practical Nurse in good standing
- Mandatory experience in palliative care
- Preferred post-graduate education in palliative care
- Valid driver's license and access to a vehicle required
- Experience in community care is an asset
- Reliability and flexibility are essential
- Willingness to travel to patient residences within a designated area
- Experience working with vulnerable populations
- A passion for making a positive impact in the community
- Required on-call coverage, including after-hours, to ensure patient safety and meet care needs
