About the Role
We are seeking a skilled Palliative Care Nurse to join our team at ParaMed Home Health Care. As a Palliative Care Nurse, you will play a vital role in providing compassionate care to patients in their homes, helping them to remain comfortable and independent.
Key Responsibilities
- Conduct thorough assessments of patients' needs and develop individualized care plans
- Deliver high-quality care to patients in their homes, including medication management and wound care
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination
- Maintain accurate and up-to-date patient records
Requirements
- Registration with the CNO as a Registered Nurse in good standing
- Valid driver's license and access to a vehicle
- Palliative care experience is mandatory
- Post-graduate education in Palliative care is strongly preferred
- Experience in delivering community care is an asset
