Palliative Care Nurse Specialist

About the Role:
We are seeking a compassionate and skilled Palliative Care Registered Nurse to join our team at ParaMed Home Health Care. As a Palliative Care Registered Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their homes.
Key Responsibilities:
- Conduct thorough assessments of patients' needs and develop individualized care plans to ensure optimal comfort and quality of life.
- Deliver high-quality patient care, including administering medications, wound care, and other treatments as needed.
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
- Provide education and support to patients and families on palliative care, disease management, and symptom management.
- Document patient progress, care plans, and other relevant information in a timely and accurate manner.
Requirements:
- Current registration with the College of Nurses of Ontario (CNO) as a Registered Nurse in good standing.
- Valid driver's license and access to a reliable vehicle.
- Palliative care experience is mandatory, with a strong preference for post-graduate education in palliative care.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work independently and as part of a team, with a strong commitment to providing high-quality patient care.
