About the Role

We are seeking a skilled Palliative Registered Nurse (RN) to join our team in providing compassionate care to patients in their homes. As a Palliative Nurse, you will work with patients of all ages, delivering scheduled visits to ensure their comfort and well-being.

Key Responsibilities

  • Assess patients' individualized needs using appropriate assessment tools and techniques
  • Execute and continuously monitor care plans to ensure optimal patient outcomes
  • Engage patients using a collaborative approach, building rapport and trust to deliver high-quality care
  • Maintain and update professional patient records related to patient encounters and plans

Requirements

  • Registration with the CNO as a Registered Nurse in good standing
  • Valid driver's license and access to a vehicle
  • Palliative care experience is mandatory
  • Post-graduate education in Palliative care is strongly preferred
  • Experience in delivering community care is an asset
  • Reliability and flexibility
  • Willingness to travel to and from patient residences within a designated area
  • Passion for making a difference in your community
  • Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs
