We are seeking a skilled and compassionate Registered Practical Nurse (RPN) to join our Palliative Care team at ParaMed Home Health Care. As a Palliative Nurse, you will play a vital role in providing high-quality care to patients in their homes, focusing on comfort, dignity, and quality of life.
Key Responsibilities- Participate in scheduled visits in the community to assess and address patient needs within your scope of practice.
- Provide safe and effective nursing care, using appropriate assessment tools and techniques to identify individual patient needs.
- Develop and implement care plans, monitoring patient progress and adjusting plans as required.
- Build strong relationships with patients and their families, promoting trust and open communication.
- Maintain accurate and up-to-date patient records, documenting all encounters and care plans.
- Registration with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse in good standing.
- Palliative care experience is mandatory; post-graduate education in Palliative care is strongly preferred.
- Valid driver's license and access to a vehicle are required.
- Experience in delivering community care is an asset.
- Reliability, flexibility, and a willingness to travel to and from patient residences within a designated area.
- Experience working with vulnerable populations and a passion for making a difference in your community.
- Ability to work independently and as part of a team, with a strong commitment to patient-centered care.
