Job SummaryWe are seeking an experienced Community Outreach Worker to join our team in Prince George. As a key member of our team, you will provide outreach support to clients referred by our funders and community partners. Your primary responsibility will be to assist clients in maintaining housing stability through an integrated service provision approach.
Key Responsibilities- Conduct referral consultations based on criteria, urgency, and other factors to facilitate a successful transfer or referral process.
- Work with clients to identify their needs and develop a mutually agreed intervention and support plan with measurable short- and long-term goals.
- Ensure daily living aspects, including housing, nutrition, clothing, hygiene, leisure lifestyle, employment, education, and access, are addressed in planning.
- Provide coordinated case management services, physical health, mental wellness, substance use supports, social, emotional, and community supports, personal care and living supports, food security supports, indigenous cultural supports.
- Graduate with a Diploma or Certificate in a relevant field with one year of related experience.
- Demonstrated ability to respond well in crisis situations.
- Experience working with clients with a history of conflict with the law, developmental disabilities, and/or spectrum disorders.
- Understanding of providing service to people with different learning abilities.
- Extensive knowledge of community services in Northern British Columbia.
The starting wage for this position is $27.54 per hour, plus benefits enrollment as per the Collective Agreement. We offer a competitive benefits package, including health, wellness, and pension plans.
