Care Coordinator

Job Summary:
Bayshore HealthCare is seeking a skilled Care Coordinator to join our team in Halifax, NS. As a Care Coordinator, you will play a vital role in providing case management services to patients with complex healthcare needs.
Key Responsibilities:
- Assess and evaluate patient healthcare needs, developing comprehensive care plans to meet individual goals.
- Monitor patient medical treatment plans, initiating case management action plans as needed.
- Identify healthcare and community services supports required to meet patient needs.
- Participate in multidisciplinary team meetings to share patient progress and provide recommendations.
- Educate patients and families about their health conditions, providing information on benefits and entitlements.
- Develop and maintain partnerships with internal and external community organizations to streamline patient care and expand community resources.
Requirements:
- Minimum of three years of experience in case management or a related field.
- Baccalaureate Degree in Nursing from a recognized university.
- Current and valid Registered Nurse license in the province of Nova Scotia.
- Certificate of Basic Life Support (BLS) for Healthcare Provider or equivalent.
