About the Opportunity

The Care Coordinator is responsible for assessing, planning, and authorizing services for clients referred to Continuing Care. This role involves working with interprofessional teams to facilitate hospital discharge and return to the community. Care Coordinators help provide an array of services to assist individuals and families cope with complicated health situations in the community.

Key responsibilities include assessing clients' needs, identifying goals, and developing plans to meet those goals. Care Coordinators also authorize services, make referrals, and link clients with other professional and volunteer resources. They work with clients and their care partners to articulate and advocate for alternative solutions to address unmet needs.

About You

We are seeking a skilled and compassionate Care Coordinator to join our team. The ideal candidate will have a Bachelor's degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies, and current registration with the applicable Nova Scotia Professional Regulatory body. A minimum of three years' experience in community-based care management is required.

The successful candidate will possess excellent assessment, interviewing, and negotiation skills, as well as the ability to work independently and co-operatively in a busy multidisciplinary situation. They will also be competent with computer use, keyboarding, and able to conduct assessments using a laptop. Knowledge of community agencies and resources is an asset.

Hours and Compensation

This is a permanent full-time position, with 70 hours bi-weekly. The hourly rate is $41.06 - $46.75. Successful candidates may be eligible for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
