Job Summary

Vancouver Coastal Health seeks a highly motivated and compassionate Nurse Practitioner to join our Vancouver Community Palliative Care team.

This role offers the opportunity to work in a collaborative and collegial team of physicians, nurses, and allied health staff, providing exceptional care to patients and their families. Opportunities for various full-time equivalents (FTE), mentorship, participation in research, quality improvement, and education are available.

About the Role
  • Work collaboratively as an integral part of a multidisciplinary team to deliver comprehensive patient care.
  • Develop and implement population-based strategies to improve health outcomes.
  • Collaborate with clients and other healthcare professionals to identify and assess trends and patterns that impact client populations.
Responsibilities
  1. Assess patients' physical and emotional needs, develop individualized care plans, and provide evidence-based interventions.
  2. Promote healthy behaviors, educate patients and their families about disease management, and support self-care practices.
  3. Participate in peer review, program evaluation, and quality assurance activities to ensure optimal patient care.
Qualifications
  • Certification as a Nurse Practitioner with the British Columbia College of Nurses and Midwives (BCCNM).
  • Four years of recent, relevant clinical nursing experience.
  • Successful completion of the Objective Structured Clinical Examination (OSCE).
