Job Title Overview
We are seeking a dedicated and compassionate Palliative Care Specialist to join our team at Providence Health Care. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality care to patients and their families.
About the Role
The Palliative Care Specialist will work collaboratively with healthcare professionals to assess, plan, implement, and evaluate patient care. This will involve developing and implementing individualized care plans, conducting thorough assessments, and coordinating care with other healthcare teams.
Responsibilities
- Assessment and Planning: Conduct thorough assessments of patients' physical, social, spiritual, and psychological needs, and develop individualized care plans to meet these needs.
- Care Coordination: Work collaboratively with other healthcare teams to coordinate care, including medications, treatments, and other interventions.
- Communication: Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
- Clinical Decision-Making: Make informed clinical decisions based on assessment findings and evidence-based practice guidelines.
- Educational Support: Provide education and support to patients and their families to promote health and well-being.
- Documentation: Maintain accurate and timely documentation of patient care, including assessments, plans, and progress notes.
Qualifications
- Education: Registration with the BC College of Nurses & Midwives as a practicing RN registrant or registration with the BC College of Nurses & Midwives as a practicing RPN registrant.
- Skills and Abilities:
- Effective Communication: Ability to communicate effectively with patients, families, and healthcare professionals.
- Clinical Knowledge: Demonstrated knowledge of palliative care principles and practices.
- Critical Thinking: Ability to make informed clinical decisions based on assessment findings and evidence-based practice guidelines.
- Teamwork: Ability to work collaboratively with other healthcare teams to coordinate care.
Salary
$85,000 - $110,000 per annum, depending on experience.
