The Healthcare Social Worker is a key member of the interdisciplinary healthcare team, providing collaborative and holistic care on the primary to quaternary care continuum. This role is focused on the relationships between individuals and their environments, which are integral to psychosocial, emotional, and spiritual well-being. The social determinants of health are embedded in social work theory and practice, and are the focus for the dynamic and interactive process of engagement, assessment, planning, intervention, and evaluation.
About YouWe are seeking a highly skilled and experienced Social Worker to join our team. The ideal candidate will have:
- A Master of Social Work degree from an accredited university, or a Bachelor of Social Work degree with relevant experience
- Registration or eligibility for registration with the Nova Scotia College of Social Workers
- A minimum of two years of clinical experience, with a strong understanding of community resources and the ability to work independently as part of a multidisciplinary team
- Excellent communication and interpersonal skills, with the ability to coordinate and develop care plans for hospital discharge and safe transition back to the community or institutional setting
- Demonstrated use of innovative problem-solving and evidence-based practice to patient services
- Knowledge of Joint Discharge Operations Committee and its purpose
- Competencies in other languages an asset, with French preferred
This is a Casual Relief position, with a competitive hourly rate of $49.05. Successful candidates may be eligible for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
