About the Role

The Registered Practical Nurse is a key member of our community health team, providing high-quality care to clients in their homes and community settings. As a Registered Practical Nurse, you will work collaboratively with our interdisciplinary team to deliver client-centered care, promoting health, independence, and well-being.

Key Responsibilities
  • Provide physical assessments, pain and symptom management, health teaching to individuals and families, palliative care, wound care, and other services as required.
  • Work collaboratively with the healthcare team to develop and implement individualized care plans.
  • Conduct home visits to assess client needs and provide care and support.
  • Collaborate with community health teams to provide client-centered health care in homes, community settings, and clinics.
  • Participate in ongoing education and professional development to maintain and enhance clinical skills and knowledge.

Requirements
  • Must be licensed as a Registered Practical Nurse in the province of practice.
  • Experience in community health is an asset.
  • A valid driver's license and access to a reliable vehicle are required.
  • A current and original copy of a satisfactory Criminal Records Check is required and a Vulnerable Sector Search and/or a Child Abuse Registry Check may be required.
  • Must be able to wear Personal Protective Equipment (PPE).
  • On-call shifts may be required.
Working Conditions and Physical Demands

This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying (using proper techniques), bending, reaching, kneeling, and other movements that emphasize good body mechanics. Individuals in the role are required to walk, sit, stand, and climb stairs throughout the day, with some tasks requiring fine hand movements.
