Job Overview:

We are seeking a part-time Licensed Practical Nurse (LPN) to join our team as a Care Coordinator. In this role, you will work collaboratively with our experienced care team to provide clinical oversight and support to our residents.

Key Responsibilities:

  • Collaborate with the case manager from Island Health to update residents' care plans
  • Support residents in their daily routines
  • Lead and guide the care team on best clinical practices
  • Ensure documentation is completed in accordance with care standards and regulations
  • Work alternating shifts between Friday, Saturday, and Sunday (morning or evening)

Requirements:

  • Graduate of a recognized school of nursing
  • Current registration in good standing with BCCNM
  • Licensed Practical Nursing Diploma or Registered Nurse designation
  • Experience in long-term care or assisted living preferred
  • Availability to work consistent shifts over the weekend

Benefits:

  • Hourly rate ranging from $32.84 - 41.35 per hour as per the BCGEU collective agreement
