We are seeking a highly skilled Mental Health Therapist to join our team in the Tertiary Older Adult Acute program at Fraser Health Authority.

As a Mental Health Therapist, you will be part of a multidisciplinary team providing comprehensive mental health interventions and social work services to older adults with complex needs.

Job Summary

The successful candidate will provide individualized therapy sessions, develop care plans, and collaborate with healthcare professionals to ensure seamless transitions for patients.

You will have the opportunity to work closely with patients, families, and healthcare teams to address social, emotional, and cultural needs, promoting recovery and well-being.

Responsibilities
  • Provide comprehensive therapeutic mental health interventions, including crisis counseling and group work.
  • Develop, implement, and evaluate psycho-social care plans in collaboration with patients, families, and interdisciplinary teams.
  • Conduct thorough assessments of patients' social history, strengths, and resources, identifying areas for intervention and support.
  • Maintain accurate patient records, documenting progress notes, discharge summaries, and other reports as required.
  • Participate in ongoing professional development, staying current with best practices and research in mental health and geriatric care.
Qualifications

To be successful in this role, you will possess:

  • A Bachelor's degree in Social Work from an accredited institution.
  • At least two years of experience in mental health, preferably working with older adults.
  • Current full registration with the BC College of Social Workers.
  • CPR Level C certification and a valid BC Driver's License.
