About the Role:
This is a Full-Time, Temporary 6 months contract. 8am-8pm, incudes alternating weekends, with a total of 84 hours biweekly. This position would require travel between our Guelph & Cambridge clinic.
Key Responsibilities:
- Using appropriate assessment tools/techniques to assess patients' individualized needs
- Executing and continuously monitor care plans
- Engaging patients using a collaborative approach by building rapport and trust
- Maintaining and update professional patient records related to patient encounters and plans
Requirements:
- Registration with the CNO as a Registered Nurse in good standing
- Valid driver's license and access to a vehicle is required
- Experience in delivering community care is an asset
- Reliability and flexibility
- Willingness to travel to and from patient residences within a designated area
- Passion for making a difference in your community
- Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs
