About the Role

We are seeking a highly skilled and compassionate Intensive Case Manager to join our team at Good Shepherd Centres. As a key member of our housing support team, you will play a vital role in providing respectful and appropriate housing support services to individuals and families experiencing chronic homelessness.

Key Responsibilities
  • Provide trauma-informed intensive case management to youth, women, and families in need
  • Work collaboratively with service users to secure permanent housing and provide ongoing case management supports for up to 24 months
  • Partner with various agencies to leverage and coordinate assets and services available in the community to ensure that service users have their individualized housing and support needs met
  • Accompany service users to community appointments, apartment viewings, and attend service users' homes for monthly home visits
  • Must be willing and able to assist service users in a hands-on capacity
  • Time management skills are necessary in a high-energy, rapidly paced environment
  • Ability to work independently and as part of an interdisciplinary team
Requirements
  • Post-secondary education in Social Service or a related field is required
  • Minimum 1-year related experience or equivalent combination of education, training, and experience in Social Services or a related field
  • Strong knowledge of and experience working with a range of issues impacting youth, women, and families, including homelessness, violence against women and children, poverty, mental illness, and substance use
  • Commitment to and experience working within a person-centered, trauma-informed, harm reduction, and housing first framework
  • Excellent interpersonal and conflict resolution skills, verbal and written communication, time management, and organization, and ability to prioritize workload
  • Ability to work cooperatively and collaboratively within an interdisciplinary team
  • CPR certification or willingness to be certified is required
  • Valid G license required
  • Fluency in a 2nd language is an asset
