Community Care Leader

At VON Canada, we are seeking a dedicated Community Care Leader to join our team. In this role, you will be responsible for providing professional and practice leadership to staff, ensuring the provision of safe, effective quality client care within your program area(s). You will work as part of an integrated Regional management team to ensure the successful execution of site operational goals and manage the delivery of in-home client care services for Nova Scotia Home Support programs and/or community programs within Community Support Services.
Key Responsibilities:- Directs and monitors the delivery of client care services and day-to-day functions and performance of front-line staff in accordance with current regulatory guidelines and VON policies, procedures and best practices.
- Assists staff or completes risk assessments, investigations of issues, injuries, and reporting related to client or employee safety.
- Monitors performance related to funder expectations regularly and contributes to the development and implementation of quality improvement activities to achieve performance targets through monitoring of reported outcomes.
In collaboration with internal stakeholders, you will develop team procedures and guidelines to ensure best practices are met in the completion of all tasks or responsibilities. This includes receiving and documenting all employee, family, and client complaints or issues related to service delivery and escalating, if required, to the appropriate authorities.
You will also participate in budget preparation, financial performance analysis, and complete key performance indicators (KPI) and quality reports for internal and external stakeholders. Additionally, you will facilitate front-line staff development and growth using a team approach and working in concert with the practice leaders and educators to facilitate excellent outcomes for quality care delivery, client service, and provider safety.
Education, Designations and Experience:- Post-Secondary education in a related field.
- Registered Practical Nurse (RPN) or Licensed Practice Nurse (LPN) with current registration from a provincial regulatory body, without restrictions.
- A minimum of 5 years experience in community health care and a minimum of 2 years experience managing teams.
This role requires strong critical thinking, assessment, and problem-solving skills, as well as excellent verbal and written communication skills. Proficiency in Microsoft Office software is also essential.
