Job Title: Renal Social Worker

We are seeking a highly skilled and compassionate Renal Social Worker to join our team at Fraser Health Authority. As a key member of our interdisciplinary team, you will play a vital role in providing comprehensive social work services to patients and families affected by renal disease.

Job Summary

The successful candidate will be responsible for conducting thorough assessments, developing and implementing care plans, and providing education and support to patients and families. You will work closely with our healthcare team to ensure seamless care coordination and effective communication.

Responsibilities
  • Conduct comprehensive social work assessments of patients and families to identify their needs and develop personalized care plans.
  • Provide education and support to patients and families on renal disease management, treatment options, and self-care strategies.
  • Collaborate with our healthcare team to ensure timely and effective care coordination, including referrals to community resources and services.
  • Develop and maintain relationships with community agencies and organizations to ensure access to resources and services for patients and families.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Qualifications

We are seeking a candidate with a Master's degree in Social Work from an approved school of Social Work and two years of recent related clinical social work experience working with renal patients and families. Current full registration with the BC College of Social Workers is also required.

The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment. You will be a team player with a strong commitment to providing high-quality patient care and a passion for working with patients and families affected by renal disease.
