About the Role:
We are seeking a skilled Social Worker to join our Renal Program team at Fraser Health. As a key member of our interdisciplinary team, you will provide psycho-social assessments and treatment for clients and families in the Renal Program to maintain and improve their quality of life.
Key Responsibilities:- Perform psycho-social assessments and formulate treatment plans by reviewing referrals and gathering information about clients and their significant others.
- Provide clinical and ongoing supportive counseling services to clients and their families regarding identified problems, adjustment to illness, and lifestyle changes.
- Offer education services to individuals, couples, families, or groups to assist them in dealing with the psycho-social implications of treatment modalities and lifestyle changes.
- Organize and coordinate peer support programs for assigned clients.
- Provide consultative and direct services to clients, families, and staff regarding community and rehabilitation services/resources and access to these resources.
- Participate in quality improvement activities by providing input into the development, implementation, and evaluation of departmental and program goals, objectives, policies, and procedures.
- Prepare statistics for submission to designated personnel.
- Participate in department or program-sponsored research through various methods such as literature searches, administering surveys, and analyzing data.
- Bachelor's degree in Social Work from an approved school of Social Work.
- Two years' recent related clinical experience working with clients in an acute care and/or community health care environment, or an equivalent combination of education, training, and experience.
- Current full registration with the BC College of Social Workers.
- Valid BC Driver's License and access to a personal vehicle for business-related purposes.
