Job Summary

Fraser Health is seeking a skilled Social Worker to join our Renal Program team at the Royal City Centre - Kidney Care Centre in New Westminster, BC. As a Social Worker, you will play a vital role in providing psycho-social assessments and treatment to clients and families, promoting their quality of life and well-being.

Responsibilities
  1. Conduct comprehensive psycho-social assessments to identify client needs and develop individualized treatment plans.
  2. Provide clinical and supportive counseling services to clients and families, addressing issues related to illness, adjustment, and lifestyle changes.
  3. Offer education and support to individuals, couples, families, or groups, helping them navigate the psycho-social implications of treatment and lifestyle changes.
  4. Collaborate with interdisciplinary teams to develop comprehensive care plans and provide consultative services to clients, families, and staff.
  5. Participate in quality improvement activities, contributing to the development and evaluation of departmental and program goals, objectives, policies, and procedures.
Qualifications

To be successful in this role, you will require:

  • A Bachelor's degree in Social Work from an approved school of Social Work.
  • Two years of recent related clinical experience working with clients in an acute care and/or community health care environment.
  • Current full registration with the BC College of Social Workers.
  • A valid BC Driver's License and access to a personal vehicle for business-related purposes.
