Job Title: Renal Social Worker - Clinical Services Specialist
Job Summary:
We are seeking a highly skilled Renal Social Worker - Clinical Services Specialist to join our team at Fraser Health. As a key member of our interdisciplinary team, you will play a vital role in providing comprehensive social work, psychosocial, and clinical services to patients and families affected by renal disease.
Key Responsibilities:
- Provide comprehensive therapeutic counseling services to explore emotional, psycho-social responses to identify problems, personal concerns, and treatment objectives.
- Facilitate changes in coping style, behavior, attitude, and feelings.
- Coordinate services, manage cases, and facilitate meetings with patients, families, community supports, and interdisciplinary teams.
- Conduct comprehensive social work assessments of patients and families by interviewing, gathering social data, and interpreting data using various modalities.
- Provide consultative and direct clinical services to patients, families, and healthcare professionals on high-risk social factors related to adaptation to chronic illness and self-care.
- Lead the interdisciplinary team in client care planning and discharge planning from hospital stay/caseload.
- Maintain patient records by documenting assessments, charting patient information, and preparing progress notes and reports.
Qualifications:
- Master's degree in Social Work from an approved school of Social Work.
- Two years' recent related clinical social work experience working with renal patients/families/clients in an acute care and/or community environment.
- Current full registration with the BC College of Social Workers.
- Comprehensive knowledge and understanding of bio-psychosocial issues related to chronic illnesses.
- Demonstrated counseling skills, including crisis management and adjustment counseling, therapeutic interventions, grief and loss, decision making, and health transition support.
- Ability to educate and provide consultation to patients, families, and healthcare professionals.
- Ability to develop, coordinate, and deliver in-services, presentations, psycho-educational, and support groups.
- Ability to provide leadership and work effectively within an interdisciplinary team.
- Effective verbal and written communication skills.
- Ability to collaborate and work effectively in a diverse, multi-cultural, socioeconomic, demographic, and geographical environment.
- Ability to organize and prioritize work.
- Ability to operate related equipment, including computer software.
