Job Title: Renal Social Worker

We are seeking a skilled Renal Social Worker to join our team at Fraser Health Authority. As a Renal Social Worker, you will play a vital role in providing comprehensive social work services to patients and families affected by renal disease.

Key Responsibilities:
  • Conduct comprehensive social work assessments of patients and families to identify their needs and develop effective care plans.
  • Provide counseling and therapy services to patients and families to help them cope with the emotional and psychological impact of renal disease.
  • Collaborate with healthcare professionals to develop and implement individualized care plans that address the physical, emotional, and social needs of patients and families.
  • Provide education and support to patients and families to help them manage their renal disease and make informed decisions about their care.
  • Develop and maintain relationships with community resources and services to ensure that patients and families have access to the support they need.
Requirements:
  • Master's degree in Social Work from an approved school of Social Work.
  • Two years' recent related clinical social work experience working with renal patients and families.
  • Current full registration with the BC College of Social Workers.
