Key Responsibilities:- Conduct comprehensive social work assessments of patients and families to identify their needs and develop effective care plans.
- Provide counseling and therapy services to patients and families to help them cope with the emotional and psychological impact of renal disease.
- Collaborate with healthcare professionals to develop and implement individualized care plans that address the physical, emotional, and social needs of patients and families.
- Provide education and support to patients and families to help them manage their renal disease and make informed decisions about their care.
- Develop and maintain relationships with community resources and services to ensure that patients and families have access to the support they need.
- Master's degree in Social Work from an approved school of Social Work.
- Two years' recent related clinical social work experience working with renal patients and families.
- Current full registration with the BC College of Social Workers.
