About the RoleThe Primary Care LPN will be responsible for providing clinical follow-up, assessing patient needs, and developing personalized care plans. You will work closely with patients, families, and healthcare teams to ensure seamless care coordination and optimal health outcomes.
Key Responsibilities- Provide competent, safe, and ethical care to patients in various settings, including physician practices, homes, schools, and communities.
- Assess patient needs, develop care plans, and implement interventions to promote optimal health and well-being.
- Collaborate with healthcare teams to ensure continuity of care and effective communication.
- Administer medications, perform assessments, and provide personal and nursing care to patients.
- Operate related equipment, including proficiency in relevant computer applications.
- Graduation from a recognized program for Practical Nurses with one (1) year of recent, related experience; or an equivalent combination of education, training, and experience.
- Registration with BC College of Nurses and Midwives as a practicing LPN registrant (BCCNM).
- Current, valid BC Driver's License.
