Key Responsibilities:

  • Conduct scheduled community visits to address patient needs within your professional scope
  • Deliver ethical and safe nursing care
  • Utilize appropriate assessment tools and techniques to evaluate individual patient needs
  • Implement and continuously monitor care plans, making adjustments as necessary
  • Foster patient engagement through a collaborative approach, building rapport and trust
  • Maintain and update professional patient records related to encounters and care plans

Qualifications:

  • Registration with the CNO as a Registered Practical Nurse in good standing
  • A valid driver's license and access to a vehicle are required
  • Experience in community care is advantageous
  • Reliability and adaptability are essential
  • Willingness to travel to patient residences within a designated area
  • Experience working with vulnerable populations
  • A passion for making a positive impact in the community
