Advanced Practice Nurse

The Advanced Practice Nurse at Halton Healthcare is a highly skilled Registered Nurse with specialized training and expertise to autonomously assess, interpret diagnostic tests, prescribe treatments, and execute procedures within their professional scope. This position focuses on comprehensive patient care, wellness promotion, and teamwork with patients, families, and the broader community.
Key Responsibilities:
- Perform thorough health evaluations
- Work collaboratively with the healthcare team
- Create and execute individualized patient care strategies
- Offer health education and emotional support
- Oversee quality improvement initiatives
- Facilitate educational programs
- Participate in patient education efforts
- Promote research within clinical settings
Required Qualifications:
- Registered as an Extended Class Nurse Practitioner
- Master's Degree in Nursing
- Preferred eligibility for cross-appointment
- Membership in relevant professional organizations
- At least 5 years of nursing experience
- Prior experience as a Nurse Practitioner in Internal Medicine
- Strong leadership and organizational capabilities
- Proficient in Microsoft Office Suite
- Ability to work independently
