Job Title: Registered Practical Nurse

We are seeking a skilled Registered Practical Nurse to join our team in the community setting. As a Registered Practical Nurse, you will be responsible for providing high-quality patient care and working collaboratively with our team to achieve excellent outcomes.

About the Role:
  • Provide comprehensive client assessments, develop care plans, and implement and evaluate care based on best evidence and practices.
  • Support clients through care transitions by collaborating and connecting them with community support services.
  • Work collaboratively with other care team members and support functions to ensure seamless care delivery.

Requirements:
  • Current registration with the College of Nurses of Ontario.
  • Canadian Nurses Association (CNA) specialty certification an asset.
  • Enthusiasm and love of community care and passion for evidence-informed practice and excellence in care.
  • A current CPR certificate and valid driver's license.
