We are seeking a Registered Practical Nurse (RPN) to work overnight on-call in the community in Windsor, Ontario.

This is a part-time position with a competitive rate paid per client visit, flexible schedules, paid training, and great benefits.

The ideal candidate will have a passion for community care, evidence-informed practice, and excellence in care, with a current registration with the College of Nurses of Ontario and a valid driver's license and vehicle.

The successful candidate will be responsible for completing comprehensive client assessments, developing care plans, implementing and evaluating care based on best evidence/practices and on client's unique needs.

They will also support the client through care transitions by collaborating and connecting the client with community support services in their local community.

Key responsibilities:

  • Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client's unique needs.
  • Support the client through care transitions by collaborating and connecting the client with community support services in their local community.
  • Develop and implement care plans that meet the unique needs of each client.
  • Collaborate with other care team members and support functions to ensure seamless care delivery.

Requirements:

  • Current registration with the College of Nurses of Ontario.
  • Canadian Nurses Association (CNA) specialty certification an asset.
  • Enthusiasm and love of community care and passion for evidence-informed practice & excellence in care.
  • A current CPR certificate is required.
  • A valid driver's license and vehicle (travel required).
  • Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
  • A willingness to attain required Saint Elizabeth certifications.
  • Availability to work days, evenings, nights and every other weekend.
