Minimum Qualifications: Must possess a valid license and current registration as a registered professional nurse in New York State.


Duties Description:

  • Assess and monitor the medical and health requirements of individuals.
  • Conduct weekly visits to assigned residences to ensure nursing oversight and thorough evaluation of care items.
  • Collaborate with primary care physicians, medical specialists, and various clinical personnel as necessary.
  • Act as an advocate for the healthcare needs of individuals.
  • Maintain continuous communication with the medical liaison or designated personnel.
  • Support residential staff in the provision of health services for individuals.
  • Utilize appropriate reporting mechanisms and the chain of command to address individual and/or staff concerns.
  • Complete follow-up documentation for individuals after emergency room or hospital visits.
  • Review, annotate, initial, and date lab results, x-rays, and consultations, and follow up with practitioners as needed.
  • Ensure timely documentation of home visit outcomes.
  • Adhere to pressure wound policies and documentation standards.

Provide essential nursing services and oversight to Residential/Day Hab staff.



  • Respond promptly to emergency requests, both within and outside of your caseload.
  • Conduct nursing assessments and related documentation, including RN notes and nursing logs, thoroughly and within established time frames.
  • Deliver individualized nursing interventions tailored to specific needs.
  • Address staff inquiries regarding individual-specific medical and nursing matters.
  • Ensure Plans of Nursing Services (PONS) are current and signed by staff, maintaining them in electronic files.
  • Offer health-related training and consultation to day program and residential staff.
  • Annually review the medical section of Individualized Service Plans (ISP) and conduct medication audits for each home.
  • Stay informed about updates to the medical section of the ISP.
  • Obtain informed consent for medications from individuals, in accordance with established policies.

Provide adequate nursing assessment and supervision of Direct Support Assistants' competency in individual-specific nursing delegated procedures, including medication administration.



  • Complete initial medication certification and perform annual clinical performance evaluations for Direct Support Assistants regarding procedures, including medication administration.
  • Deliver training to all staff as necessary on new or revised medication policies.
  • Provide timely certification and recertification information for the database.
  • Document medication errors and implement staff remediation as required.

Offer clinical supervision to Licensed Practical Nurses.



  • Provide adequate nursing supervision and complete performance evaluations regarding individual-specific nursing procedures.

Maintain professional skills through ongoing education and training.



  • Participate in Nursing Discipline meetings.
  • Complete mandated training and pursue available discipline-specific training.
  • Identify areas for improvement within the nursing department and contribute to action plans.
  • Engage in special projects and assignments as needed.
