As a Licensed Practical Nurse (LPN) within the Reactivation Care Centre, your primary focus will be to deliver exceptional patient and family-centered care in a nurturing and culturally aware environment. You will collaborate with a multidisciplinary team of healthcare professionals, emphasizing assessment, education, prevention, maintenance, and the restoration of patient health.Key Responsibilities
In this role, you will:
Uphold the highest standards of nursing practice as outlined by the regulatory body. Communicate effectively, both verbally and in writing, with patients, families, and healthcare team members. Employ critical thinking skills to prioritize and organize tasks based on patient needs. Demonstrate proficiency in safe patient handling techniques. Commit to continuous professional development and quality improvement initiatives. Utilize computer systems for data entry and patient information management. Exhibit self-motivation in pursuing further education related to patient assessment and geriatric care. Apply strong assessment skills with a focus on the geriatric population. Showcase effective organizational, problem-solving, and time-management abilities. Foster positive relationships in a family-centered care environment.
Qualifications
To be considered for this position, you must possess:
A diploma in Practical Nursing from an accredited institution. Registration and good standing with the regulatory nursing body. Current Basic Cardiac Life Support (BCLS) certification. A minimum of two years of recent clinical experience, particularly with older adults and those with cognitive impairments. Completion of a recent health assessment course. Bilingual proficiency in French and English is an asset.
