As a Community Support Facilitator, you will participate in enjoyable community activities, foster meaningful connections, and positively influence the lives of individuals and their families.

PRIMARY RESPONSIBILITIES:

  • Assist clients in creating and assessing personal plans and objectives.
  • Identify progressive steps towards achieving set roles and goals.
  • Ensure a supportive and engaging atmosphere for clients.
  • Encourage team development within individual teams, across different teams, inter-agency, intra-agency, and within the community.
  • Provide personal care as necessary.
  • Implement behavior support and adhere to established behavior plans as required.
  • Support community inclusion by accompanying adults to registered recreational or social events; promote positive community relationships.
  • Assist clients in accessing resources related to augmentative communication when needed.
  • Facilitate the development of relationships and natural support systems.
  • Attend all mandatory workshops and re-certifications.
  • Complete necessary documentation within specified timeframes (e.g., Case Notes, Medical Administration Reports, Incident Reports, Timesheets, etc.).
  • Participate in all required meetings and planning sessions as requested.
  • Maintain a thorough understanding of the 'Community Resource Worker Position Profile.'
  • Stay informed of and comply with agency policies and procedures; work in accordance with Creating Excellence Together (CET) Accreditation Standards.

QUALIFICATIONS:

  • Disability studies certificate/diploma or similar educational background or equivalent experience, with a minimum of a Grade 12 High School Diploma.
  • Current Standard First Aid CPR/AED – Level C certification is mandatory.
  • A valid driver's license (and access to a vehicle) is advantageous.
  • Proficiency in MS Office and Outlook is required.
  • Exceptional written and verbal communication skills.
  • Engagement in self-development activities to ensure continuous learning and uphold high professional standards.
  • Knowledge of or experience in community inclusion practices.
  • Ability to provide support to adult clients effectively.
  • Capacity to identify advocacy needs and represent clients and individuals with disabilities.
  • Ability to coordinate and access appropriate services and supports for clients.
  • Experience in providing personal care is essential.
  • Experience in lifting and transferring clients is an asset.
  • Experience in supporting clients with challenging behaviors is beneficial.
