About the Role

We are seeking a highly skilled and compassionate Mental Health Support Worker to join our team at Heritage Nanny Community Living and Support Inc. As a key member of our community living and support team, you will play a vital role in providing support and services to individuals with mental health needs.

Key Responsibilities
  • Case Management: Develop and implement individualized support plans to meet the unique needs of our clients.
  • Assessment and Intervention: Conduct thorough assessments to identify client needs and develop strategies for intervention and support.
  • Community Linkage: Establish and maintain relationships with community resources and services to ensure seamless support for our clients.
  • Client Support: Provide emotional support, guidance, and education to clients to promote independence and self-advocacy.
  • Documentation and Reporting: Maintain accurate and comprehensive records of client interactions, progress, and outcomes.
  • Team Collaboration: Work collaboratively with our multidisciplinary team to ensure comprehensive support and services for our clients.
Requirements
  • Education: College/CEGEP diploma in a related field (e.g., social work, psychology, or a related field).
  • Experience: 2 years to less than 3 years of experience working with individuals with mental health needs.
  • Skills: Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Certifications: Valid certification in crisis intervention, first aid, and CPR.
