About the Role

We are seeking a highly skilled and compassionate Mental Health Support Worker to join our team at Heritage Nanny Community Living and Support Inc. As a key member of our community living and support team, you will play a vital role in providing support and services to individuals with mental health needs.

Key Responsibilities
  • Case Management: Develop and implement individualized support plans to meet the unique needs of our clients.
  • Assessment and Intervention: Conduct thorough assessments to identify client needs and develop strategies to address them.
  • Support and Guidance: Provide emotional support and guidance to clients, promoting independence and self-advocacy.
  • Community Linkage: Establish and maintain relationships with community resources and services to ensure clients receive comprehensive support.
  • Documentation and Reporting: Maintain accurate and timely records of client interactions, progress, and outcomes.
Requirements
  • Education: College/CEGEP diploma or equivalent in a related field.
  • Experience: 2 years of experience working with individuals with mental health needs.
  • Skills: Excellent communication and interpersonal skills, ability to work independently and as part of a team.
Preferred Qualifications
  • Certifications: Certification in mental health support work or a related field.
  • Training: Training in crisis intervention, suicide prevention, and cultural competency.
