About the Role

We are seeking a highly skilled and compassionate Mental Health Support Worker to join our team at Heritage Nanny Community Living and Support Inc. As a key member of our community living and support team, you will play a vital role in providing support and services to individuals with mental health needs.

Key Responsibilities
  • Case Management: Develop and implement individualized support plans to meet the unique needs of our clients.
  • Client Support: Provide emotional support, guidance, and advocacy to clients in a non-judgmental and empathetic manner.
  • Community Integration: Assist clients in accessing community resources, services, and activities to promote social inclusion and independence.
  • Communication: Maintain effective communication with clients, families, and other stakeholders to ensure seamless service delivery.
  • Documentation: Accurately document client progress, incidents, and other relevant information to support service evaluation and improvement.
  • Team Collaboration: Work collaboratively with other team members to ensure comprehensive and coordinated service delivery.
  • Professional Development: Participate in ongoing training and professional development to enhance skills and knowledge in mental health support and community living.
Requirements
  • Education: College/CEGEP diploma in a related field (e.g., social work, psychology, or a related field).
  • Experience: 2 years to less than 3 years of experience in a mental health support or community living setting.
  • Skills: Excellent communication and interpersonal skills, ability to work independently and as part of a team, and a strong commitment to providing person-centered support.
