Southbridge Care Homes is currently in search of a caring, passionate, and motivated Registered Practical Nurse with expertise in Behavioral Support and Retirement Home services. This individual will play a vital role in ensuring the safety, health, and happiness of our residents. Reporting to the Director of Care and Assistant Director of Care, the BSO - RPN will provide behavioral support to residents, demonstrating knowledge in dementia care and mental health challenges. Responsibilities include collaborating with various stakeholders to develop a person-centered care philosophy.

As a BSO - RPN, your responsibilities will include:

  • Delivering person-centered care that enhances the abilities and self-esteem of residents while developing strategies to improve their quality of life.
  • Conducting assessments using various tools to observe and identify residents' behaviors and associated risks.
  • Ensuring care plans are aligned with residents' responsive behaviors and conducting regular rounds to mitigate risks.
  • Communicating with external stakeholders and management to address risks related to responsive behaviors.
  • Participating in quality improvement initiatives and providing monthly reports on progress.
  • Offering coaching and mentorship to staff to foster collaborative care for residents exhibiting responsive behaviors.
  • Conducting reviews of resident care and attending quality program meetings.
  • Collaborating with pharmacists and physicians regarding the impact of medication on residents.
  • Engaging in staff development programs and ongoing training to enhance skills in managing responsive behaviors.
  • Assisting with tasks while adhering to home policies and the Behavioral Support Ontario project.
  • Maintaining strong relationships with all stakeholders, including staff, residents, and the community.
  • Providing excellent customer service to all stakeholders and assisting with administrative duties as needed.
  • Performing other related duties as required.

The ideal candidate will possess:

  • A current certificate of competence from the College of Nurses of Ontario.
  • A minimum of two years of experience in a similar role, preferably in long-term care, with comprehensive knowledge of responsibilities.
  • A degree in human psychology or behavioral sciences is considered an asset.
  • Bilingualism in English and French is advantageous but not mandatory.
  • A genuine passion for caring for seniors.
  • Registration or eligibility for registration with the Registered Practical Nurses Association of Ontario.
  • The ability to work independently and collaboratively, prioritizing tasks effectively.
  • Excellent verbal and written communication skills.
  • Strong creative problem-solving abilities to consider the broader context.
