Southbridge Care Homes is actively searching for a dedicated, compassionate, and enthusiastic Registered Practical Nurse with a focus on Behavioral Support and Retirement Home services to join our exceptional team. In this role, you will play a vital part in ensuring the safety, health, and happiness of our cherished residents. Reporting to the Director of Care and Assistant Director of Care, the BSO – RPN will be instrumental in providing behavioral support, demonstrating expertise in dementia care and mental health challenges.

As a BSO – RPN, your responsibilities will include:

  • Delivering person-centered care that enhances the abilities and self-esteem of residents, while developing strategies to promote their optimal quality of life.
  • Conducting thorough assessments utilizing various tools to identify resident behaviors, behavioral risks, and caregiver strategies.
  • Ensuring care plans are aligned with the responsive behaviors of residents, conducting regular evaluations to mitigate risks.
  • Collaborating with external stakeholders and the management team to address risks associated with responsive behaviors.
  • Participating in quality improvement initiatives and providing monthly performance reports.
  • Offering coaching and mentorship to staff to foster collaborative care and services for residents exhibiting responsive behaviors.
  • Conducting reviews of resident care and attending quality program meetings.
  • Working closely with pharmacists and physicians to consult on the effects of medication treatments.
  • Engaging in staff development programs and ongoing training to enhance skills in managing responsive behaviors.
  • Assisting with tasks while adhering to home policies and the Behavioral Support Ontario project.
  • Maintaining strong relationships with all stakeholders, including staff, residents, and the community.
  • Providing excellent customer service to all stakeholders and assisting with administrative duties as needed.
  • Performing other related duties as required.

The ideal candidate will possess the following qualifications and skills:

  • Current certificate of competence from the College of Nurses of Ontario.
  • A minimum of two years of relevant experience, preferably in a long-term care environment, with a comprehensive understanding of the role.
  • A degree in human psychology or behavioral sciences is considered an asset.
  • Bilingualism in English and French is advantageous but not mandatory.
  • A genuine passion for caring for seniors and a commitment to service.
  • Registration or eligibility for registration with the Registered Practical Nurses Association of Ontario.
  • Ability to work independently and collaboratively within a team, prioritizing tasks effectively.
  • Strong communication skills, both verbal and written, along with excellent comprehension abilities.
  • Developed problem-solving skills with a focus on the overall picture.
