About the Role:

We are seeking a compassionate and skilled Registered Practical Nurse to join our team at Southbridge Care Homes. As a Behavioral Support Specialist, you will play a vital role in providing person-centered care to our residents, promoting their abilities and self-esteem, and developing strategies to enhance their quality of life.

Key Responsibilities:

  • Conduct comprehensive assessments of residents' behavioral needs, identifying risks and developing strategies to mitigate them.
  • Collaborate with the care team to develop and implement person-centered care plans, ensuring the best possible outcomes for our residents.
  • Communicate effectively with residents, families, and healthcare professionals to ensure seamless care coordination.
  • Participate in quality improvement initiatives, providing feedback and suggestions for enhancing our care services.
  • Provide coaching and mentorship to staff members, promoting a culture of excellence and teamwork.
  • Stay up-to-date with industry developments and best practices, applying this knowledge to improve our care services.

Requirements:

  • Current certificate of competence from the College of Nurses of Ontario.
  • Minimum two years of experience in a similar role, preferably in long-term care, with a strong understanding of behavioral support principles.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Ability to prioritize tasks, manage multiple responsibilities, and maintain a high level of professionalism in a fast-paced environment.
