Southbridge Care Homes is seeking a compassionate and dedicated Registered Practical Nurse (RPN) with experience in both Behavioral Support Nursing (BSO) and Retirement Home care to join our team at Seaforth LTC and Retirement Home.

As a BSO – RPN, you will play a vital role in providing person-centered care to our residents. You will contribute to their well-being by promoting their abilities, fostering self-esteem, and developing strategies to enhance their quality of life. Your expertise in dementia care and mental health issues will be invaluable in supporting residents with responsive behaviors.

Responsibilities:
  • Deliver person-centered care by promoting resident abilities and self-esteem, and developing strategies to optimize their quality of life.
  • Conduct comprehensive assessments using various tools to identify behavioral risks and caregiver strategies.
  • Ensure the plan of care effectively addresses resident responsive behaviors, mitigating risks through weekly rounds and collaboration with staff.
  • Communicate with stakeholders and management teams to identify and address risks associated with responsive behaviors.
  • Participate in quality improvement activities and provide monthly reports on progress.
  • Coach and mentor staff to ensure collaborative care and services for residents with responsive behaviors.
  • Conduct resident care reviews and attend quality program meetings, including P.A.C. sessions.
  • Collaborate with pharmacists and physicians to discuss the impact of medication treatment on residents.
  • Engage in staff development programs and seminars, maintaining ongoing training to enhance skills in managing residents with responsive behaviors.
  • Adhere to home policies and the Behavioral Support Ontario project guidelines.
  • Cultivate positive relationships with all stakeholders, including staff, residents, and the general public.
  • Provide exceptional customer service to all stakeholders and assist with administrative duties as required.
Qualifications:
  • Current certificate of competence from the College of Nurses of Ontario.
  • Minimum two years of experience in a similar role, preferably in long-term care, with comprehensive knowledge of duties.
  • Degree in human psychology or behavioral sciences is an asset.
  • Bilingualism (English and French) is an asset but not required.
  • A genuine passion for caring for seniors.
  • Registration/eligibility for registration with the Registered Practical Nurses Association of Ontario.
  • Ability to work independently and as part of a team, prioritizing tasks effectively.
  • Excellent communication (verbal and written) and comprehension skills.
  • Strong creative problem-solving skills and a holistic perspective.
