About the Role:
We are seeking a compassionate and skilled Registered Practical Nurse to join our team at Southbridge Care Homes. As a Behavioral Support Specialist, you will play a vital role in providing person-centered care to our residents, promoting their well-being and quality of life.
Key Responsibilities:
- Conduct comprehensive assessments of residents' behavioral needs and develop individualized care plans to address these needs.
- Collaborate with the interdisciplinary team to provide responsive care and mitigate risks associated with behavioral issues.
- Communicate effectively with residents, families, and healthcare professionals to ensure seamless care coordination.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures related to behavioral support.
- Provide coaching and mentorship to staff members to enhance their skills and knowledge in behavioral support.
- Conduct regular care reviews and attend quality program meetings to ensure the highest standards of care are maintained.
Requirements:
- Current certificate of competence from the College of Nurses of Ontario.
- Minimum two years of experience in a similar role, preferably in long-term care, with comprehensive knowledge of duties.
- Strong communication and interpersonal skills, with the ability to work effectively in a team environment.
- Ability to prioritize tasks and manage multiple responsibilities in a fast-paced environment.
- Registration/eligible for registration with the Registered Practical Nurses Association of Ontario.
