About the Opportunity

We are seeking a highly skilled and dedicated Wellness Facilitator Physiotherapist to join our team at Nova Scotia Health Authority. As a key member of our Hants Health and Wellness Team, you will play a vital role in providing wellness programming and navigation, chronic disease management, nutrition education, and emotional wellness counselling to our patients.

The ideal candidate will have a strong background in physiotherapy and experience working with individuals with chronic diseases in an exercise program or setting. You will also have excellent communication and interpersonal skills, with the ability to build strong relationships with community groups and partners.

We are looking for a team player who is passionate about population health, health promotion strategies, and behaviour change principles. If you have a strong understanding of local health and community-based services, and are able to demonstrate person-centered/family-focused, diversity-focused approaches to care, we want to hear from you.

Key Responsibilities:

  • Provide wellness programming and navigation, chronic disease management, nutrition education, and emotional wellness counselling to patients
  • Collaborate with community groups and partners to build trusting relationships and develop projects/initiatives that benefit the public
  • Develop and implement individual and group education/facilitation programs utilizing adult education, self-management supports, and motivational enhancement principles
  • Conduct assessments and develop plans to support patients with chronic diseases
  • Work with interdisciplinary teams to provide comprehensive care to patients

Requirements:

  • Bachelor's degree in physiotherapy from an accredited university
  • Membership with the Nova Scotia College of Physiotherapists
  • Eligibility for membership with the Canadian Physiotherapy Association
  • Minimum one year experience in each of the following: working with individuals with chronic diseases in an exercise program or setting, working with community groups/partners building trusting relationships, and individual and group education/facilitation
  • Understanding of population health, health promotion strategies, and behaviour change principles
  • In-depth knowledge of local health and community-based services
  • Demonstrates person-centered/family-focused, diversity-focused approaches to care
  • Well-developed interpersonal, teamwork, oral and written communication skills
  • Demonstrates flexibility, adaptability, and willingness to change
