About the Opportunity

We are seeking a highly skilled and dedicated Physiotherapist to join our Hants Health and Wellness Team (HHWT) as a Wellness Facilitator-Physiotherapist. This is an exciting opportunity to work in a community-based, integrated chronic disease prevention and management service that provides wellness programming and navigation, chronic disease management, nutrition education, and emotional wellness counselling.

The HHWT is an interdisciplinary team that builds community connections and partnerships to provide programs and services that are responsive to community needs. As a Wellness Facilitator-Physiotherapist, you will use critical thinking and clinical judgment to support patients (in both individual and group-based settings) with the management of their chronic disease and wellness.

You will collaborate with HHWT staff, primary care providers, other health care providers, and community groups to maintain awareness of existing supports within the community and surrounding areas. This work is done through a community development, population health, and collaborative approach with emphasis on self-management supports.

About You

We are looking for a candidate with a Bachelor's degree in physiotherapy from an accredited university and membership with the Nova Scotia College of Physiotherapists. You must be eligible for membership with the Canadian Physiotherapy Association and have a minimum of one year experience in each of the following:

  • Working with people with chronic disease in an exercise program or setting
  • Working with community groups/partners building trusting relationships through working together on projects/initiatives/partnerships that benefit the public
  • Individual and group education/facilitation utilizing adult education, self-management supports, and motivational enhancement principles

You should have an understanding of population health, health promotion strategies, and behaviour change principles, as well as in-depth knowledge of local health and community-based services. You must demonstrate person-centered/family-focused, diversity-focused approaches to care, and have well-developed interpersonal, teamwork, oral, and written communication skills.

We are looking for a candidate who is flexible, adaptable, and willing to change, and who demonstrates innovation in program planning, implementation, and evaluation, and quality improvement, planning skills. You should have computer skills (MS Office, email, and internet) and be able to travel within the community with a valid Nova Scotia driver's license and daily access to a reliable vehicle.

Hours and Compensation

This is a permanent full-time position with 75 hours biweekly. Shifts may include evenings, weekends, and holidays. The hourly rate is $33.96-$42.89.
