Indigenous Chronic Kidney Disease Care Lead

We are seeking a highly skilled and dedicated professional to fill the role of Indigenous Chronic Kidney Disease Care Lead at Windsor Regional Hospital. This position plays a vital role in supporting the delivery of renal care services to Indigenous communities in the region.
Key Responsibilities- Collaboration and Partnerships
- Establish and maintain relationships with regional Indigenous partners and healthcare providers.
- Communicate regularly with hospital leadership and regional partners on Indigenous health initiatives.
- Provide support and expert consultation to healthcare providers to facilitate access to care and services.
- Represent the hospital in provincial quality initiatives and committees focused on Indigenous health.
- Indigenous Supportive and Palliative Care Services
- Provide support to Indigenous patients with chronic kidney disease and their families by facilitating access and coordination of services.
- Initiate referrals to appropriate personnel and advocate on behalf of patients and families.
- Individualize patient care pathways to reflect interventions that meet identified needs while promoting cultural safety and appropriateness.
- Review referral processes and recommend improvements to increase access to renal services.
- Support regional and provincial initiatives to improve the detection and prevention of CKD in Indigenous communities.
- Develop a regional inventory of renal care programs and services for Indigenous patients and families.
- Monitor and record services provided for Indigenous patients and families.
- Document patient experiences and develop a work plan to address support issues and cultural needs.
- Support palliative care research and innovation projects.
- Support continuing professional development programs in Indigenous care for healthcare professionals.
- Support program evaluations by providing input into Indigenous health strategic priorities and sharing best practices.
- Operations
- Participate in the development of the hospital's work plan and identify key activities, deliverables, and performance measures.
- Complete and submit reports on engagement with Indigenous partners as required.
- Participate as an active member of the hospital's leadership team.
- University degree in a health or social sciences field, or college diploma with five years' work experience in the health and/or social services field.
- Indigenous lived experience is preferred.
- Completion of Indigenous cultural competency training.
- Palliative Care for Front Line Workers in First Nations Communities training is an asset.
- Learning Essential Approaches to Palliative and End of Life Care (LEAP) training is an asset.
- Experience working with Indigenous communities and knowledge of Indigenous history, culture, and health issues.
- Knowledge of the healthcare environment and the renal system, as well as the renal journey for Indigenous people.
- Knowledge of culturally safe care and its application in a healthcare environment.
- A valid Ontario Driver's License and access to a reliable vehicle is required.
- Fluency in one or more Indigenous languages is considered an asset.
