Job Summary

VON Canada is seeking a skilled Clinical Care Specialist to provide medical care to children, youth, adults, and seniors in their homes and other settings in communities across Nova Scotia.

Key Responsibilities

  • Provide medical care to patients in their homes and other settings
  • Manage diabetes, dialysis, IV medications, IV/CVAD care, palliative care, phlebotomy, and wound care
  • Work with patients to develop and implement care plans
  • Collaborate with healthcare teams to ensure comprehensive care
  • Document patient information and progress

Requirements

  • Licensed as a Registered Nurse in the province of practice
  • Experience in community health an asset
  • Valid driver's license and access to a reliable vehicle required
  • Criminal Records Check and Vulnerable Sector Search required
  • Ability to wear Personal Protective Equipment (PPE)

Working Conditions

This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying, bending, reaching, kneeling, and other movements that emphasize good body mechanics.
