Position Overview

The Care Coordination Specialist plays a crucial role in evaluating, planning, and managing care for clients referred to Continuing Care services. This position is integral in providing a spectrum of programs and services aimed at assisting individuals and families navigate complex health challenges effectively within the community.

Key Responsibilities

The Care Coordinator is tasked with:

  • Conducting thorough assessments to identify client needs and goals.
  • Developing tailored care plans in collaboration with clients and their support networks.
  • Authorizing necessary services and facilitating referrals to appropriate resources.
  • Working closely with multidisciplinary teams to ensure smooth transitions from hospital to home.
  • Maintaining ongoing communication with clients, families, and healthcare providers to ensure continuity of care.
  • Managing financial responsibilities related to authorized services in accordance with established policies.
  • Conducting regular reassessments to adapt care plans as needed.
Qualifications

Ideal candidates will possess:

  • A Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies.
  • Current registration or eligibility for registration with the relevant professional regulatory body.
  • A minimum of three years of experience in a related field.
  • Demonstrated expertise in community or long-term care settings.
  • Experience in case management or discharge planning.
  • Proficiency in computer skills and the ability to conduct assessments using digital tools.
  • Knowledge of community resources and agencies.
  • Strong assessment, interviewing, and negotiation abilities.
  • Excellent verbal and written communication skills.
  • The capacity to work both independently and collaboratively in a dynamic environment.
  • Effective time management and prioritization skills.
  • A valid driver's license and access to a reliable vehicle.
  • Competency in additional languages is an asset, with French being particularly valuable.
Work Schedule

This is a full-time position requiring 70 hours bi-weekly, with shifts that may include days, evenings, weekends, and holidays.
