Client Care Facilitator

The Care Coordination Specialist plays a crucial role in evaluating, planning, and managing care for clients referred to Continuing Care services. This position is integral in providing a spectrum of programs and services aimed at assisting individuals and families navigate complex health challenges effectively within the community.
Key ResponsibilitiesThe Care Coordinator is tasked with:
- Conducting thorough assessments to identify client needs and goals.
- Developing tailored care plans in collaboration with clients and their support networks.
- Authorizing necessary services and facilitating referrals to appropriate resources.
- Working closely with multidisciplinary teams to ensure smooth transitions from hospital to home.
- Maintaining ongoing communication with clients, families, and healthcare providers to ensure continuity of care.
- Managing financial responsibilities related to authorized services in accordance with established policies.
- Conducting regular reassessments to adapt care plans as needed.
Ideal candidates will possess:
- A Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies.
- Current registration or eligibility for registration with the relevant professional regulatory body.
- A minimum of three years of experience in a related field.
- Demonstrated expertise in community or long-term care settings.
- Experience in case management or discharge planning.
- Proficiency in computer skills and the ability to conduct assessments using digital tools.
- Knowledge of community resources and agencies.
- Strong assessment, interviewing, and negotiation abilities.
- Excellent verbal and written communication skills.
- The capacity to work both independently and collaboratively in a dynamic environment.
- Effective time management and prioritization skills.
- A valid driver's license and access to a reliable vehicle.
- Competency in additional languages is an asset, with French being particularly valuable.
This is a full-time position requiring 70 hours bi-weekly, with shifts that may include days, evenings, weekends, and holidays.
