About the Opportunity

The Continuing Care Coordinator is responsible for assessing, planning, and authorizing services for clients referred to Continuing Care. This role involves working with interprofessional teams to facilitate hospital discharge and return to the community.

The Care Coordinator will assess clients' needs, identify goals, and develop plans to meet those goals. They will also authorize services and make referrals to other resources to support community-dwelling clients. This role requires strong assessment, interviewing, and negotiation skills, as well as excellent verbal and written communication skills.

The ideal candidate will have a Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies, and current registration with the applicable Nova Scotia Professional Regulatory body. They will also have a minimum of three years' experience in a community or long-term care setting, with knowledge of community agencies and resources.

The Continuing Care Coordinator will work independently and collaboratively in a busy multidisciplinary environment, prioritizing workload and managing time efficiently. They will also be responsible for reassessments at routine intervals and skilled care management.

About You

We are seeking a highly skilled and experienced professional to join our team as a Continuing Care Coordinator. The ideal candidate will have:

  • Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies
  • Current registration with the applicable Nova Scotia Professional Regulatory body
  • Minimum three years' experience in a community or long-term care setting
  • Demonstrated knowledge and/or experience in dedicated case management or discharge planning
  • Competent with computer use, keyboarding, and able to conduct assessments using a laptop
  • Knowledge of community agencies and resources
  • Excellent assessment, interviewing, and negotiation skills
  • Excellent verbal and written communication skills
  • Ability to work independently and collaboratively in a busy multidisciplinary environment
  • Ability to prioritize workload and manage time efficiently
  • Valid driver's license and access to a reliable vehicle
  • Competencies in other languages preferred; French an asset
Hours and Compensation

This is a permanent full-time position, working 70 hours bi-weekly. Shifts may include days, evenings, weekends, and holidays. The successful candidate will be compensated at a rate of $46.75 per hour.
