Position Overview

The Care Coordination Specialist plays a crucial role in evaluating, planning, and managing care for clients referred to Continuing Care services. This position is integral in assisting individuals and families navigate complex health challenges, ensuring they receive the appropriate support in their community.

Key Responsibilities

The primary responsibilities include:

  • Conducting thorough assessments to identify client needs and goals.
  • Developing tailored care plans in collaboration with clients and their support networks.
  • Authorizing necessary services and facilitating referrals to additional resources.
  • Working closely with multidisciplinary teams to ensure smooth transitions from hospital to home.
  • Maintaining ongoing communication with clients, families, and healthcare providers to support effective care management.
Qualifications

We invite candidates who possess the following qualifications:

  • A Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies.
  • Current registration or eligibility for registration with the relevant professional regulatory body.
  • A minimum of three years of experience in a relevant field.
  • Proven knowledge and experience in community or long-term care settings.
  • Familiarity with case management and discharge planning processes.
  • Proficient in computer usage and capable of conducting assessments using digital tools.
  • Strong understanding of community resources and agencies.
  • Exceptional communication, assessment, and negotiation skills.
  • Ability to work independently and collaboratively in a dynamic environment.
  • Valid driver's license and access to a reliable vehicle.
  • Competency in additional languages is an asset; proficiency in French is preferred.
Work Schedule

This is a full-time position requiring 70 hours bi-weekly, with shifts that may include days, evenings, weekends, and holidays.

Compensation and Benefits

The hourly wage for this position is $46.75. Successful candidates may be eligible for a comprehensive benefits package, including health, dental, travel, long-term disability, and life insurance, as well as a defined benefit pension plan.
