Position Overview

The Care Coordination Specialist plays a crucial role in the assessment, planning, and management of care for clients referred to Continuing Care services. This position involves collaborating with clients and their families to navigate complex health situations, ensuring they receive the appropriate support and resources in their community.

Key Responsibilities

The primary responsibilities of the Care Coordination Specialist include:

  • Conducting comprehensive assessments to identify client needs and goals.
  • Developing personalized care plans in collaboration with clients and their support networks.
  • Authorizing services and making referrals to relevant community resources and professionals.
  • Facilitating transitions from hospital to home, ensuring a smooth discharge process.
  • Maintaining ongoing communication with clients, families, and healthcare teams to provide continuous support.
  • Managing care resources responsibly within established policies and guidelines.
Qualifications

Ideal candidates will possess the following qualifications:

  • A Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies.
  • Current registration or eligibility for registration with the appropriate professional regulatory body.
  • A minimum of three years of relevant experience in a community or long-term care setting.
  • Demonstrated expertise in case management or discharge planning.
  • Proficiency in computer usage and conducting assessments digitally.
  • Strong knowledge of community agencies and available resources.
  • Exceptional assessment, communication, and negotiation skills.
  • Ability to work independently and collaboratively in a fast-paced environment.
  • Valid driver's license and access to a reliable vehicle.
  • Competencies in additional languages are an asset.
Work Schedule

This is a permanent full-time position requiring 70 hours bi-weekly, with shifts that may include days, evenings, weekends, and holidays.
