About the Opportunity

The Care Coordinator is responsible for assessing, planning, and authorizing services to support clients with complex health needs in the community. This role involves working with interprofessional teams to facilitate hospital discharge and ensure a smooth transition to community-based care.

The Care Coordinator will assess clients' needs, identify goals, and develop a plan to meet those goals. They will also authorize services and make referrals to other resources as needed. This role requires strong communication and negotiation skills, as well as the ability to work independently and collaboratively in a fast-paced environment.

The ideal candidate will have a strong understanding of community resources and services, as well as experience working with clients with complex health needs. They will be able to work effectively with clients, families, and healthcare teams to achieve positive outcomes.

About You

We are seeking a highly skilled and compassionate Care Coordinator to join our team. The ideal candidate will have:

  • A Bachelor's degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies
  • Current registration with the applicable Nova Scotia Professional Regulatory body
  • Minimum three years' experience in a community or long-term care setting
  • Demonstrated knowledge and experience in case management or discharge planning
  • Excellent assessment, interviewing, and negotiation skills
  • Strong communication and interpersonal skills
  • Ability to work independently and collaboratively in a fast-paced environment
  • Valid driver's license and access to a reliable vehicle
Hours and Compensation

This is a permanent full-time position, working 70 hours bi-weekly. The hourly rate is $46.75. Successful candidates may be eligible for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
