Position Overview
As a Community Health Worker, you will collaborate with a dynamic team of Nurses and LPNs to support clients' independence and ability to remain in their homes. Community Health Workers deliver home support services by assisting with personal care and daily living activities while promoting maximum independence. They play a crucial role in the home health care plan for each client.

Key Responsibilities
  • Providing daily personal care, including bathing, dressing, grooming, and oral hygiene
  • Performing household tasks to maintain a safe and healthy home environment
  • Preparing meals according to established plans
  • Assisting with mobility and transfers
  • Monitoring and reporting changes in client behavior and condition
  • Executing delegated tasks in accordance with training and established procedures
This is a Relief Part Time (0.90 fte) opportunity until the return of the incumbent. The shift types for this position include rotating days and evenings with hours from 07:00 to 14:30, 07:00 to 15:30, and 15:00 to 23:00.
