Position Overview
As a Community Health Worker, you will collaborate with a dynamic team of Nurses and LPNs to support the independence of Community Clients, enabling them to remain in their homes. Community Health Workers deliver home support services, assisting with personal care and daily living activities while promoting maximum independence. You will play a vital role in the home health care plan tailored for each client.

Key Responsibilities
  • Assist with daily personal care activities, including bathing, dressing, grooming, and oral hygiene
  • Perform household tasks to maintain a safe and healthy living environment
  • Prepare meals according to established dietary plans
  • Provide assistance with mobility and transfers
  • Monitor and report changes in client behavior and condition
  • Execute delegated tasks in accordance with training and established procedures
This is a Relief Part-Time (0.90 FTE) opportunity until the return of the incumbent. The shifts for this position will vary between days and evenings, with hours ranging from 07:00 to 14:30, 07:00 to 15:30, and 15:00 to 23:00.
